Firearms Forms

The following forms are provided by the Pennsylvania State Police in agreement with the Uniform Firearms Act. Read carefully and if you have a question contact the Firearm Records Unit at 717-783-5495.

Application/Record of Sale Form (SP4-113)

This form will be provided by the Pennsylvania State Police and all requests for this form must be submitted in writing. Fax requests to 717-772-4249 or mail requests to:

Note the pre-printed numbers on this form are assigned to your dealership. You can not loan copies to other dealers or duplicate this form. Please allow several weeks for processing your order. 

Information for Pennsylvania Firearm Purchasers & Basic Firearm Safety (SP4-135)

The Firearms Safety Brochure will be provided by the Pennsylvania State Police. You can fax your requests to 717-772-4249 or mail requests to:

Forms Available Online to be Reproduced by Dealer

Surcharge Remittance Form (SP4-134)

This form may be duplicated as needed. The most current version of this form is posted on the PSP web site. All previous versions shall be destroyed.

In agreement with 18 PA C.S. § 6111, this form shall be submitted within 14 days of a sale, transfer, or instantaneous records check. The licensee/sheriff shall enter the date span in the designated space on the form, e.g., 01/01/00 to 01/14/00. The form shall include all sales, transfers, or instantaneous record checks made during that time period, e.g., from 01/01/00 to 01/14/00.

If you have a date span that includes more than one calendar year, you must submit a separate form for each calendar year. Never list a date span that includes more than one year.  Instructions for dealers.

Surcharge Remittance Continuation Form (SP4-134A)

NOTE: A new version of this form was posted in October 2020. To maximize space for handwritten entries, select 'Actual Size' under Page Sizing & Handling in the print window.

This form may be duplicated as needed. The most current version of this form is posted on the PSP web site. All previous versions shall be destroyed. Instructions for dealers.

Pennsylvania Instant Check System Challenge Form (SP4-197)

This form may be duplicated, however, all four (4) pages of this form must be submitted for a challenge to be accepted as complete. The most current version of this form is posted on the PSP web site. All previous versions shall be destroyed.
